| Project description |
| The Acibadem Healthcare Group, a leading private provider of healthcare services in Turkey and an existing IFC client since 2004, has applied to IFC for a corporate loan of up to US$40 million. The purpose of this new project is to expand their healthcare facilities in a number of major cities in Turkey, including Istanbul and Izmir. The total investment program for the period from 2006 to 2008 is estimated to be about US$100 million, which will include a combination of acquisition of existing hospitals and the construction of new hospitals and outpatient clinics. Currently Acibadem, which is affiliated with Harvard Medical International, operates five full-service hospitals, five outpatient clinics, one ophthalmology clinic, and one central laboratory. |

| Environmental and social issues |
This is a Category B project according to IFC’s Procedure for Environmental and Social Review of Projects because a limited number of specific environmental and social impacts may result which can be avoided or mitigated by adhering to generally recognized performance standards, guidelines or design criteria. The review of this project consisted of appraising technical, environmental, health and safety, as well as social/community information submitted by the project sponsor as well as a site visit to the construction site of their Acibadem’s new Bursa Hospital. The following potential environment, health and safety (EHS), and social/community impacts of the projects were analyzed:
Development and implementation of corporate EHS management systems by Acibadem;
Compliance of Acibadem’s existing and new facilities with Turkish and IFC EHS requirements;
Commitment of Acibadem to quality and efficiency and to EHS performance including international certification; and
Acibadem’s commitment to be a responsible corporate citizen.

| Proposed mitigation for environmental and social issues |
The project sponsor, Acibadem, has presented plans to address these impacts to ensure that the proposed project will upon implementation of the specific agreed measures, comply with the environmental, health and safety, and social/community requirements of Turkey as well as the IFC environment and social policies and the environmental, health and safety guidelines. Information about how these potential impacts will be addressed by the sponsor/project is summarized below.
Development and implementation of corporate environmental, health and safety (EHS) management systems
Acibadem has agreed to hire a full-time corporate Environment, Health and Safety professional at a very senior level. Acibadem also needs to develop and implement a corporate Life and Fire Safety (L&FS) Master Plan which can then be used to develop and implement a L&FS Master Plan for each Acibadem facility. Acibadem needs to develop and implement a corporate Health Care Waste Management System (HWMS) which can then be implemented at each Acibadem facility. Finally Acibadem needs to develop and implement a corporate Occupational Health and Safety Management System (OHSMS), which can also then be implemented at each Acibadem facility. Prior to first disbursement Acibadem has committed to (1) hire the EHS senior manager; (2) develop and implement their corporate L&FS Master Plan which is acceptable to IFC; (3) develop and implement their corporate HWMS which is acceptable to IFC; and (4) develop and implement their corporate OHSMS which is acceptable to IFC.
Compliance of existing and new facilities with Turkish and IFC EHS requirements
Wastewater from each hospital will be pretreated prior to discharge to the municipal sewer system. The nature and extent of this wastewater pretreatment will depend largely on the degree of treatment provided by the municipal wastewater treatment systems. Acibadem will endeavor to ensure that the requisite municipal wastewater treatment systems are being operated effectively and in a manner to ensure that the final treated wastewater meets both IFC and Turkish requirements. Where available each hospital will use natural gas as a fuel source and where natural gas is not available will discuss alternative fuel sources with IFC. Again all air emissions will comply with both IFC and Turkish requirements. Solid, infectious and hazardous wastes from all Acibadem facilities will be handled and disposed of according to the IFC approved HWMS. Solid, infectious and hazardous wastes must be disposed of in Government of Turkey licensed municipal or other facilities. Acibadem will again ensure that these facilities are being operated effectively and in compliance with IFC and Turkish requirements. All Acibadem’s facilities will comply with the requirements in their IFC approved corporate Life and Fire Safety Master Plan.
Commitment to quality and efficiency
Acibadem has developed a world class system of policies and procedures that cover all aspects of hospital operation. Since October 2003 Acibadem has been affiliated with Harvard Medical International, a foundation supported by Harvard Medical School and seventeen other training hospitals and institutes. The benefits of this cooperation include
- executive professional development of medical staff, especially nurses;
- development and strengthening of quality management related to healthcare services;
- development of healthcare services and treatment protocols in oncological and neurological sciences; and
- access to new and improved treatment models and services applied in Harvard and their associated hospitals.
Acibadem’s medical services are organized through the US based Joint Commission International (JCI) accreditation standards, developed specifically for the healthcare sector, while the work for organizational standards is being carried out with the EFQM (European Foundation for Quality Management) Excellence Model. Acibadem also has achieved ISO 9001 (Quality) certification, has a policy on Patients Rights, and has extensive training for staff (e.g. technical skills development, professional training, on-the-job training).
Community responsibility
The private sector has been providing an increasing level of quality healthcare in Turkey. Recent changes to the law mean that increasing numbers of Turks can avail themselves of higher quality healthcare services provided by the private sector. This is a trend that is likely to continue in the future. Acibadem participates in a wide variety of community outreach programs including the Acibadem Health and Education Foundation which focuses on public health education and supporting students studying in the healthcare sector. Acibadem has a particular focus on healthcare for children and provides healthcare services for the underprivileged in both Istanbul and Bursa. |

| Conclusion |
| Accordingly, IFC concludes that the proposed project will meet the applicable Turkish and IFC environment and social policies and the environmental, health and safety guidelines upon successful implementation of the agreed mitigation measures. |

| Monitoring and compliance |
| IFC will evaluate the project’s compliance with the applicable Turkish and IFC environmental, health and safety and social/community requirements during the lifetime of the project by reviewing the annual monitoring reports (AMRs) prepared for the project covering the ongoing performance of project-specific environmental, health and safety (EHS) and social/community activities. Periodic site supervision visits will also be conducted. |
